HBOT for Sports Recovery and Athletic Performance
HBOT around exercise does not improve performance or ease ordinary muscle soreness, but it may speed recovery from actual muscle injury, three different outcomes with three different answers.

In short
The best current evidence gives three different answers to what sounds like one question. For performance and for recovery from ordinary exercise, HBOT used before or after a workout does not show a measurable benefit. For muscle soreness (DOMS), it also does not help overall. But for recovery from actual exercise-induced muscle injury, a 2026 meta-analysis found it did accelerate healing. The trials are small and the protocols vary widely, so this sits at an experimental evidence level, not an established one.
The clinical question
Does HBOT used around exercise improve athletic performance, reduce muscle soreness, or accelerate recovery from acute muscle injury, and does the answer differ by which of these three outcomes is asked?
What is established
Hyperbaric oxygen therapy is an established medical procedure. Breathing 100 percent oxygen at greater than atmospheric pressure, usually in a sealed chamber, sharply raises the amount of oxygen dissolved in blood plasma. This mechanism is well understood and the procedure has recognized, insurance-covered indications including decompression sickness, carbon monoxide poisoning, certain non-healing wounds and radiation injury. The chamber, the physiology and the safety profile in a medical setting are not in question.
What is being reviewed here is a different and much newer use: healthy athletes using HBOT sessions timed around training or competition, hoping it will make them perform better, hurt less afterward, or heal faster from muscle damage. That is an off-label, performance-and-recovery application, and it needs its own evidence base separate from the procedure's established medical indications.
What the evidence for sports recovery actually shows
A 2021 systematic review and meta-analysis pooled 10 studies covering 166 participants and looked specifically at HBOT given before exercise, after exercise, or during exercise. For HBOT given before or after exercise, the pooled analysis found no statistically significant effect on athletic performance and no significant effect on recovery measures. The one signal that emerged was for HBOT delivered during exercise itself, where there was a possible benefit for muscle endurance, a narrower and less commonly used protocol than the pre or post-exercise sessions most people picture.[1]
A separate 2026 meta-analysis, this one of 10 randomized controlled trials and 299 participants, split the question differently, by outcome rather than by timing. It found that HBOT significantly accelerated recovery from exercise-induced muscle injury, meaning objective measures of muscle damage and function after a genuinely damaging exercise protocol. But the same review found no significant overall benefit for delayed-onset muscle soreness, the subjective ache most athletes actually mean when they ask about recovery.[2]
Put together, these two reviews are not contradictory, they are answering different questions. Performance itself: no evidence of benefit. Ordinary soreness after a hard session: no evidence of benefit. Recovery from a documented muscle injury: a positive signal, but from trials that are still small and inconsistent in how they dosed and timed the therapy.[1][2]
Three questions, not one
"Does HBOT help sports recovery" is not a single question with a single answer. Performance, soreness and injury recovery are distinct outcomes, and the evidence answers each of them differently. Treating them as interchangeable is where most of the marketing claims around HBOT and sport go wrong.
What remains uncertain, including the negative findings
Two negative findings sit inside this evidence base and should not be quietly dropped. First, for the pre and post-exercise timing that most people mean when they ask about HBOT for sport, the 2021 meta-analysis found no significant effect on performance or recovery. Second, the larger 2026 review found no significant benefit for DOMS overall, the muscle soreness that peaks a day or two after unfamiliar or intense exercise, despite finding a benefit for the injury subgroup.[1][2]
Both reviews flag the same underlying limitation: small trials, few participants per subgroup, and wide variation in pressure, session duration, timing relative to exercise and how muscle injury or soreness was measured. Ten trials pooled across genuinely different protocols is a reasonable basis for a hypothesis, not for a firm clinical recommendation. Nothing here has been replicated in a large, protocol-standardized trial.[1][2]
What this means for you
This evidence keeps HBOT for sports recovery firmly in the experimental category at AION. It does not become a standing recommendation for athletic performance and it is not offered as a routine post-exercise recovery protocol, because the evidence for exactly that use, pre or post-exercise sessions aimed at general recovery or performance, does not show a benefit.
Where a patient has a specific, documented muscle injury and a physician judges the exercise-injury evidence to be relevant to their situation, that is a decision made individually, after diagnostics and clinical assessment, not from a fixed sports-recovery protocol applied to everyone who trains hard. No fixed number of sessions or timing schedule is presented here as proven, because none currently is.[2]
If you are considering HBOT for sport, here is the accurate picture: a well-understood medical procedure being tested in a new, still-thin evidence context, where the honest answer depends on which specific outcome you actually care about.
