In short

The clinical question
Do IV NAD+ infusions or general "wellness" vitamin drips produce measurable benefits in energy, cognition, immune resistance, hangover recovery, or aging biomarkers in healthy adults?
What is actually established
Giving fluids, electrolytes, or vitamins through an IV line is a routine, well-understood medical procedure. It is the right tool when someone is dehydrated, cannot absorb a nutrient orally, or has a measured deficiency such as low B12 or iron. None of that is in dispute, and it is not what this review is about.
NAD+ (nicotinamide adenine dinucleotide) is a real and essential coenzyme. Cells need it for energy metabolism and DNA repair, and levels do decline with age. That basic biology is well documented in laboratory and animal research. The open question is whether infusing extra NAD+ into a healthy adult's bloodstream does anything measurable for how that person feels, thinks, resists illness, or ages. That is a separate, much less settled question from the biochemistry.
"Wellness" IV drips marketed for energy, immunity, hangover relief, or anti-aging are a commercial category, not a defined clinical indication. They are typically offered without a preceding blood test to confirm a deficiency exists, which is a different model of care from IV therapy used to treat a diagnosed problem.
What the evidence for IV NAD+ specifically shows
The most directly relevant trial is a 2024 randomized, double-blind, placebo-controlled pilot study that gave healthy adults a single dose of IV NAD+ or IV nicotinamide riboside. It found the infusions were tolerable at reduced infusion rates and did raise whole-blood NAD+ levels acutely. That is the extent of what it showed. The trial was designed to test safety and pharmacokinetics, not whether people felt more energetic, thought more clearly, or got sick less often. It has not yet been peer-reviewed.[2]
A 2024 systematic review pooled ten randomized trials of NAD or NADH supplementation, involving 489 people in total. It reported that supplementation was generally well tolerated and found modest effects on anxiety symptoms and insulin sensitivity in some of the individual trials, while explicitly stating that more investigation is needed before clinical benefit claims are justified. Critically, this review is dominated by oral NADH and nicotinamide precursor studies, not IV NAD+, and the individual trials were very small.[1]
Put together, there is no published controlled trial showing that IV NAD+ improves energy, cognition, immune function, or any aging biomarker in a healthy adult. What exists is one small pilot confirming the infusion is tolerable and reaches the bloodstream, plus a review of a largely different, oral form of the compound showing tentative and modest signals in unrelated small trials.[2][1]
The trap: a marker moving is not the same as a benefit
Raising a blood level is not a clinical result
The pilot trial found that IV NAD+ raises whole-blood NAD+ concentration. That is a pharmacokinetic fact, not a health outcome. Plenty of interventions can move a number in a blood test without producing anything a patient would notice or benefit from. Until a trial measures energy, cognitive performance, illness frequency, or a validated aging outcome directly, in a placebo-controlled design, in the population actually being sold the treatment, "it raises your NAD+ level" is a marketing shortcut, not evidence of benefit.
Hangover recovery and general wellness claims
IV hangover clinics are a distinct but overlapping marketing category from NAD+ drips. The relevant systematic review here predates the current IV clinic trend, having pooled randomized controlled trials of both conventional and complementary hangover treatments. Its conclusion was blunt: there was no compelling evidence that any intervention tested, conventional or complementary, effectively prevented or treated alcohol hangover.[3]
That review did not test commercial IV hydration or vitamin drips directly, because that specific product category had not yet emerged at the time. So it cannot be read as a direct verdict on today's IV hangover offerings. What it does establish is that the broader evidence base for hangover interventions, going back two decades, has consistently failed to find a reliable fix, which should temper confidence in newer commercial variants that have not been tested any more rigorously.[3]
For the broader claims made about general wellness vitamin drips, energy, immunity, and cognition in people without a diagnosed deficiency, there is no controlled trial evidence provided or found for this review that supports those specific outcomes. The honest position is an absence of evidence for benefit in healthy adults, not evidence of no effect, since the well-designed trials that would settle the question largely have not been run on the commercial product as sold.
What remains uncertain
It remains uncertain whether IV NAD+ produces any effect beyond what oral precursors already achieve, since the two have not been compared head to head for clinical outcomes. It remains uncertain whether repeated IV NAD+ dosing over weeks or months does anything different from a single dose, since the pilot trial only tested one infusion. And it remains uncertain whether any hydration or vitamin IV changes a validated aging biomarker in a way that matters clinically, since no source reviewed here tested that question directly.[2][1]
What this means for you
AION does not offer NAD+ or general wellness IV drips as anti-aging, immunity, or hangover treatments, and does not describe any IV protocol as a fixed or standard package. Where IV therapy is used, it follows a physician's review of blood work and clinical history, targeting a specific, documented need rather than a general wellness effect.
If a patient asks about NAD+ specifically, the honest starting point is that the pharmacokinetic data exists but the outcome data does not yet. That conversation belongs with a physician who can weigh it against the patient's actual bloodwork, medications, and goals, not a menu of pre-set infusions. As more rigorous trials with real efficacy endpoints are published, this review will be updated to reflect them.
